What's new for 2024


Medical Options 1, 2 and 3 will see an increase in contributions.
  • Option 1 has lower contributions along with a reduced annual deductible and out-of-pocket maximum.
  • Option 2 has lower contributions, and as a qualified high-deductible plan, the option to open a Health Savings Account (HSA). Click here for more details on Option 2.
         - All services, including prescription drugs, are subject to the deductible, except preventive services.
 
  • Option 3 has the lowest contributions and the highest deductibles and co-insurance maximum. This option continues to offer an HSA.

When choosing an option to enroll in, consider the cost you pay in payroll contributions, the cost you pay when receiving care and the savings accounts available to you.

ATTRIBUTES
You pay higher contribution amounts in exchange for the lowest deductible and out of pocket maximum amounts.		 ATTRIBUTES

*Savings Account
HCFSA = Health Care Flexible Spending Account
LPFSA = Limited Purpose Flexible Spending Account
HSA = Health Savings Account. HSAs are available for Options 2 & 3 which can be used to set aside money on a pre-tax basis for eligible health care expenses to use today or in the future. With an HSA, the balance rolls over from year to year, and the account is yours to keep - even if you leave the Company, change to a non-high deductible health plan, or retire.

 

Coverage Information


With a few exceptions, your medical insurance benefits including behavioral health and prescription drug coverage start after you meet your annual deductible. For coverage details, call your plan administrator or refer to Your Benefits Handbook.     
  • Most medical services are covered at 80% of eligible expenses after you meet your annual deductible, if care is received from in-network providers.
  • For out-of-network care, services are covered at 60% of eligible expenses after you meet your annual deductible.*
  • Due to IRS regulations, an annual deductible applies to all services except preventive under Options 2 & 3.
  • Preventive screenings are covered at no cost, in-network under each plan option.
*Additional out-of-network provider billed charges may apply.


 
Preventive screenings and services, as recommended by the U.S. Preventive Services Task Force (USPSTF), are covered 100% in-network, not subject to the annual deductible. Medical necessity, frequency, condition, gender and/or age restrictions may apply. You should always ask your doctor if the screenings performed for or during your physical exam are consistent with the U.S. Preventive Services Task Force recommendations and subsequently covered at 100%. If you have questions, please contact your health plan administrator for additional details. Preventive services are not covered out-of-network.
